In-home support and home care for the elderly and people with disability
Helping people live independently and safely at home.
Our In-Home Support Program
Otherwise known as the Commonwealth Home Support Program, our In-home Support Program extends beyond the home to encompass support in the community.
The Commonwealth Home Support Program is funded through a federal grant plus a minimal co-contribution.
To be eligible to receive services under the CHSP Participants must be:
• Aged 65 years or older, or 50 years or older and identify as an Aboriginal and/or Torres Strait Islander person.
• Still living at home.
• Needing help at home to continue living independently.
How can we support you?
Personal care
We assist with personal hygiene, grooming, daily living tasks, and assistance with medication.
Domestic assistance
We assist with cleaning, laundry, meal preparation, shopping, and other light household chores.
Social support
Maintain social connections and engage in community activities. Program participants are also encouraged to attend our Day Programs.
Respite
We offer three types of respite as part of our In-home Support Program, including:
Centre-based respite: Providing opportunities to engage in social activities, recreational programs, and group-based support.
Flexible respite: Support to provide temporary relief for carers.
Cottage respite: Providing caregivers with a temporary break from their caregiving responsibilities while ensuring the wellbeing and safety of the client in a supportive and home-like overnight setting.
How do you get support with Monaro Community First?
Aged Care Assessment
To access services through the CHSP, participants must first register with My Aged Care, which may organise a Regional Assessment Service (RAS) home support assessment to discuss individual needs.

As part of our assessment process for new CHSP service users, a home visit will be arranged. Face-to-face assessments may take around one hour or more per person.
You are welcome to have an advocate or a trusted person attend the assessment with you. This is especially important if you feel unsure about representing yourself or are concerned about remembering, understanding, or consenting to decisions regarding services, finances, or privacy.
Care and support at home
Support at Home Packages with Monaro Community First
The Support at Home Program supports older people with complex needs to stay at home. There are four Home Care Package levels to help meet the needs of each person’s individual circumstances.
The Support at Home program is provided via a brokerage arrangement with external providers. Through the program, individuals receive funding for services covered through their provider. Using the brokerage allows for Monaro Community First’s friendly and knowledgeable team members to continue providing the service, ensuring quality and continuation of care.
This is a fee for service, and the fees are not associated with a program of funding, and are paid for through the Support at Home program and co-contribution.
